Advanced Care Planning Workshop

Whether you are 18 or 118 years old, it is important for you to express your wishes for care if you are or will become unable to advocate for yourself. Most people don’t want to think about illness or longevity, especially how we want to be cared for in times of need.

The Temple Emanuel Chesed Committee and our friends from Trellis Supportive Care are hosting a FREE workshop on April 11, 2023 (2:00 to 4:00 pm) at Temple Emanuel.

In order for you to make a more informed decision, over a couple hours we will walk through the differences between Palliative Care and Hospice along with the Advance Care Planning “legal” form. The form allows your family or designee to communicate your wishes properly if you are unable to do so yourself.

Please register so we can reserve a place for YOU. At the end of the program, you will have time to complete your documents and have them notarized on site if desired. Once you register, materials will be sent via email or you can pick up printed packets at Temple Emanuel.
